A complete solution to request a waiver for Hospice Care in your California residential care facility.
ORDER INSTRUCTIONS: Choose the number of beds you would like to request the waiver for. Please make the selection in “Number of Requested Beds”. Each waiver order is limited to a quantity of one. If a waiver is needed for an additional and different licensed facility, start a new order for that facility.
This is the Hospice Waiver + the Total Care Exception in one package. The Total Care Exception is suggested for anyone who cares for Hospice residents since in most cases the individual will need “total care”.
In your waiver request, licensing will need to know the number of beds in your facility that will be assigned to residents who are receiving hospice care. Please make the selection in “Number of Requested Beds”.
